Child Find offers information and FREE SCREENINGS for children birth to 5 years of age with medical issues or who may have difficulty with learning, speaking, behaving, seeing, walking, listening and/or sensory issues.

There are 3 ways to begin the referral process for screening. Please choose only 1 option. If you select the option to complete the Referral Form Online and you would like to print a copy for your records, just "right click" over the completed form and select "print".
